How to Apply for the Program

Step 1. 

Download and fill out the intake form and email or send it back.

Step 2. 

The intake form will be reviewed by our Clinical Manager who will call to schedule a visit.

Step 3. 

Come visit!  Bring your prospective student intern to the coffee shop.  Explore the options and learn more about the program.

Step 4.

Think you're a good fit? Fill out the enrollment form and return it to us.

Step 5.
